NOTICE

As we all prepare to take on this invisible threat, our team at AMI has been working diligently to make sure you have access to surgical care. Through Tele-Surgery, we are prepared to continue our patient visits and consultations using a secure HIPPA compliant web-based media platform. All our surgeons and physician assistants will be able to connect directly with you, face to face, to discuss and review any concerns you may have. If you need to be seen, this can be discussed through our Tele-Surgery platform and a one-one visit can be arranged in the office within 24-48hr period.

Friday                         9:00 am – 4:30 pm
Saturday, Sunday & holidays by appointment only.

We are always available for emergencies after hours and on weekends call us at 203-327-4444.

MEDICAL RECORDS

There is $20 charge for copies of all medical records. Copies must be requested in advance.

CANCELLATIONS

Cancellations of appointments must be given with at least 24hrs noticed or a $50 fee may be applied for missed appointments or cancellations less than 24 hrs in advance.

PATIENT RESPONSIBILITY

  • It is the patient’s responsibility to read and understand all permits and/or consents he/she signs. If the patient does not understand, it is the patient’s responsibility to ask the doctor or office staff for clarification.
  • It is the patient’s responsibility to answer all medical questions truthfully to the best of his/her knowledge.
  • It is the patient’s responsibility to contact our office if he/she has any complications following any procedure.
  • It is the patient’s responsibility to assure payments for services rendered are on a timely basis accepting ultimate responsibility as his/hers, regardless of whatever insurance coverage he/she may have.
